Care Plans for People Living with Diabetes
Chronic Disease Management Plans (CDMP) are designed to assist people living with a chronic medical condition.
A chronic medical condition is one that you have had for six months or longer. These include diabetes, asthma, heart problems, cancer, stroke, arthritis and depression.
Why do I need an CDMP?
These plans can help you manage your diabetes or other medical condition by identifying your health needs and planning what should be done. A GPMP plan involves you visiting your GP who, will prepare the plan which will provide an organised approach to your care. It is the plan of action that you and your GP have agreed on.
Having diabetes means that you have complex care needs. This type of care arrangement helps coordinate your care. Your GP will work with at least two other care providers involved in your care, such as a diabetes educator and podiatrist or dietitian.
Individual allied health care services
Once you have an CDMP you can be referred for up to five allied health service appointments each calendar year. Your GP will decide whether you should be referred for these services.
The five appointments can be provided by one allied health professional or shared across five different professionals. For instance, as a person with diabetes your doctor may decide you need to have five appointments with a dietitian, or two with a dietitian, one with a diabetes educator and two with a podiatrist.
You can request that your GP refers you to an allied health professional that you already know, or your GP may recommend one.
Allied health care professionals need to meet specific eligibility criteria and must be registered with Medicare Australia.
Am I eligible?
If you have a chronic medical condition such as diabetes, which is being managed by your GP under a Chronic Disease Management Plan, you will be able to receive education services from allied health professionals, specifically designed to help you manage your diabetes.
Medicare Rebate
Diabetes Tasmania charges only the Medicare rebate. You will be bulk-billed for services provided by allied health professionals employed by Diabetes Tasmania, so you will have no out-of-pocket expenses.
